It's a good thing the cities I've been to have Federal Signal, ASC, and Whelen. (Though I despise Whelen)
Why. Why do you hate Whelen? Whelen makes the best electronic sirens on the market. The ones in my town carry miles and they have broken drivers... they've been serving for over 20 years. They still work and have had little to no maintenance. One day, I was in Dublin, MD, and the Whelen 2902 in Conowingo, MD could be heard clearly running wail from there. That's over 8 miles away, and it was raining. Whelen makes pretty good sirens...
Seems like you've been watching too much TSM
The either 4004 or vortex in Tipton,MO, either broke its wires (Joke) or the rotator is broken, or it is coded to not rotate. It has been in the same position for many years now. I mostly prefer Mechanical sirens and they just make electronic sirens. I will admit that I watch a lot of TSM and Random Siren Shorts. Thanks BSP.
I’m assuming you’re a thunderbolt fanboy too… Not surprising considering you watch BSP. Anyways, Whelen is actually a great company, and what’s funny is they’re more reliable then Thinderbolts.

Heidlersburg, PA's 2T22B... the brushes were so bad on it that it sounded like it was running on 50HZ.
https://www.youtube.com/watch?v=OJCQHYwUIdA&t=206s Here's my video.
Maybe the brush angle was retarded a little during the refurb? In the videos of it doing Alert it held pitch fine.

Back on topic, I saw a video of a Model 2 with major fluctuation issues, likely with the brushes. It sounded worse than that Thunderbolt which sounded like a dying cow.
Mind posting that video?

Athens, AL 508. It's apparently running on AC power instead of it's batteries, though we don't know how TVA set the sirens up to run in 2012, it fluctuated and couldn't really hold a steady pitch until the end.

Edit: At the time the video was recorded West Shore recently did yearly maintenance on them too,

Freebrick made a video on it

The worst siren I ever heard was a Cyclone 125 in Amarillo, TX so it was testing day and the Cyclone was having problems it went off for a few seconds and stop then it went on for a few seconds then stop and it kept on repeating like that I pretty heard the Thunderbolt sounding pretty much lol.

For me, it would 100% have to be both of the Federal Signal "Moldulator's" that we have in Puyallup, Washington.
Back in late 2015 or VERY early 2016, the 6048 on Pioneer Ave had trouble starting during one of its tests. It could actually compare to the Whoop tone on a Whelen, except it never really got high pitched. It ended up starting normally about 10 seconds later. That 6048 now functions normally.
Next is the (What was once a Dual Tone) 6048 at Chief Leschi Schools. It's drivers aren't very happy, and an old video by MiltonWATech99 really proves it. Even after its upgrades in 2020, it isn't 100% working. I went to its recent test on Monday, and it was cracking up for the first 15-20 seconds.

I remeber watching a video of a 2001 (SRN I think) that had batteries so dead it barley even ran.
Found the video. https://youtu.be/8YrrC5fPCsw
