Whatever the case was with this, it has unfortunately been replaced. If you go to the next street over there's a T-128 in the 2018 street view.
Looks to me like this was a Banshee that got replaced by a T-128. No OM-117 exists in dual tone that I know of.
Clayton Werden bought the patent for both the Banshee and P-15, and most sirens installed by him were single tone. Except for a couple out in Clermont County Ohio that are dual tone.
